Admission & Registration
Stallworth Charter School recruits a diverse student population within the San Joaquin Valley. At our school, we seek to create a learning environment that will help students become lifelong learners and successful adults that benefit their family and community. We admit all students who wish to attend based on our enrollment limit. Registration Requirements
We are pleased that you are applying to Stallworth Charter School. Our goal is to make this process as smooth as possible for you and your child. In addition to filling out registration paperwork available at our school office, parents must submit the required documents and one of the following forms of identification to our office:
- Birth certificate
- Baptismal certificate
- Early school record
- Doctor’s record, post-natal care
- Hospital record of birth
- Native American tribal document
- Adoption record
- Passport
- Parent affidavit
- Proof of residency, such as a utility bill with the parent’s name and address on it
- Up-to-date immunization records

Kindergarten Requirements
In addition to the above documents, kindergarten students must have a physical exam, and parents must report it on the health check-up form or fill out the health waiver and an oral examination form.
